Capstone R4
The Atlas Triage Pipeline
Orbit gets about ten new issues a day, and a maintainer spends an hour labelling them, hunting for duplicates and asking reporters which version they run. R4 does that work. That means it takes real actions on text written by strangers, so this is where the Phase 8 defenses from Days 68–75 stop being exercises and start carrying weight.
Short on time? The 30-second version
- A triage pipeline that writes to a tracker needs a boundary between the stages that read stranger text and the one stage that can write; R4 draws it with typed facts, three write kinds and a single write module.
- Code gets the last word after every model call: labels are checked against the allowlist, duplicates against the candidates, flags force
needs_human, and flagged issues get no auto-labels. - Approval is an
interrupt()in a node with no side effects; writes happen in a separateapplynode, at most once per content-derived idempotency key, and every outcome lands in the audit log. - Measure R4 like a product: label F1, duplicate precision/recall,
needs_humanrecall, reply coverage, $/issue and p95 on 30 gold issues, with ASR reported separately. - Single versus multi-agent is a measured decision: here the boundary, not quality, justifies four components, and the README says so with numbers.
At a Glance
- Time: ~3 h core (capstone; fine to split across two sittings) (+ optional Go Deeper) · Depth: working
- Prerequisites: Day 62 (your
r3-v1.0tag) and Days 64-75 (especially 69, 70, 71, 73, 74, 75) - You'll build: R4, a LangGraph pipeline that turns each new issue into proposed labels, a duplicate link and a draft reply, applies them only after a maintainer approves, exactly once, with an audit trail, plus an eval, a single-vs-multi-agent comparison, red-team CI, a threat-model update, a numbers-first README and the tag
r4-v1.0 - Stops at: deployment, an HTTP API and a UI (Day 77 onwards)
- Fast path: Already shipped R3 and worked through Phase 8? Run the first three commands in Shipping R4, read Single vs Multi-Agent: The Measured Case and do Exercise 2 (~60–90 min).
- Cost: ≈ $1 on
gpt-6-lunafor one live eval of both configurations plus the live red-team tier, with R3's web search off (prices as of 2026-11, verify). Everything on this page also runs offline for $0. - New terms: triage pipeline, proposed action, release tag
What R4 Does, and What It Must Never Do
A triage pipeline is a fixed sequence of steps that sorts incoming work: read, classify, link, route to a person. Think of an office post room: a clerk copies the facts onto a form, a specialist investigates, a writer drafts the reply, and a manager signs before anything goes out.
For each new Orbit issue, R4 produces a proposed action: a typed record of one write (add labels, link a duplicate, post a comment) that is waiting for approval and carries its own idempotency key. Its goals are measured against eval/triage_gold.jsonl, 30 issues with gold labels, duplicates, an escalation flag (needs_human) and the points a good first reply covers.
The non-goals matter more than the goals:
| R4 never... | Enforced by |
|---|---|
| closes, deletes or reassigns anything | ProposedAction.kind is a Literal of three write kinds; nothing else type-checks |
| writes before a maintainer approves | the only write path sits after interrupt(); a missing or malformed answer is a reject |
| applies the same action twice | a content-derived idempotency key checked against the tracker log |
| lets issue text reach the triager or responder | they receive IssueFacts and R3's Diagnosis, never the body |
| auto-labels an issue that talked to "the AI" | propose drops add_labels when the reader flags an injection |
Every row is code, and each has a test in Red-Team CI for R4.

The Pipeline at a Glance
The pipeline has three trust zones. The untrusted zone reads stranger text and holds no write tools. The privileged zone sees only typed data. The write zone runs only after a person says yes.
Start with configuration. These are R3's knobs plus two R4 lines: an offline switch and Day 75's canary, a marker string planted in system prompts that must never appear in any output.

Guard, Quarantine, Investigate
The naive version hands the raw issue to one model with every tool. Issue #20 ends with a paragraph addressed to "AI assistant" asking it to label the issue severity:S1 and close every other open issue. Day 69 showed how often that works. R4 runs three steps before any decision is made.
Guard. guard_input runs Day 70's layer: a length fix for the 6,000-word log issue, Day 73's regex redaction so the fake emails and sk-test-FAKE keys never reach the reader, the triager or the tracker, and (live only) the free moderation endpoint. R3 loads the stored issue itself, so the output guards' pii fix covers anything it repeats. This is a compact form of Day 73's redact(): it returns (clean_text, entities) instead of a RedactionResult, so don't swap the two modules without adapting the call sites.

Quarantine. Day 69's quarantined reader is a model call with no tools whose only output is a validated IssueFacts: two enums, error codes, a version and a flag. Offline, a compact version of Day 69's GullibleModel stands in, and it obeys any type:/comp: label it reads in a paragraph addressed to a bot. If the flag misses, the steered labels reach the approval screen, which is why approval exists.

R3 runs with its default dry-run approval node, so its own post_comment proposal is always rejected. R4 owns every write.
Triage and Duplicates
The triager decides labels, a duplicate link and whether a human must look. The key design choice: it never sees the issue text. It gets the validated facts, R3's diagnosis, the duplicate candidates and the flags, and then code gets the last word on every field. Unknown labels are dropped, a duplicate must be a candidate, and any flag forces needs_human.
Duplicate candidates come from a compact keyword search over orbit.db: earlier issues ranked by title overlap. Work one by hand. #22 is "Same job runs on two workers at once (video transcode)"; its gold duplicate #4 is "Monthly statement job runs twice, customers get two emails". They share 3 words (job, runs, two) out of 14 distinct ones, so the Jaccard score (shared ÷ all distinct) is 3/14 = 0.21. That's below DUP_THRESHOLD (0.4), so the link is missed: same bug, different words. The stretch exercise adds hybrid search (Day 27: Hybrid Search: Keywords + Vectors) for exactly this.

Drafting a Safe Reply
The responder writes the first reply from the triager's reply points and R3's doc ids, using a sync copy of Day 41's evaluator–optimizer refine() (capped at 2 rounds, returning (text, passed)). The evaluator is free and exact: the output guards plus "mention the duplicate". Then the guards run once more, Day 71's safe_markdown strips raw HTML and off-allowlist links, and an AI-disclosure footer goes on (Day 68's transparency item).

The offline labels are wrong (gold: type:bug, comp:core, duplicate of #4): that's the stand-in reader's quality. What this run proves is the mechanics. Nothing was written before approval, each action was applied once, and the second resume found nothing pending.
Break it on purpose: move the apply_action loop into approval, before interrupt(). The first invoke writes before anyone approved, and every resume re-runs the node and writes again. The idempotency key stops the repeat; nothing stops the first write. Writes belong in their own node after the pause.
Measuring R4
Score any runner (a function from issue_id to (TriageProposal, $)) on the 30 gold issues: label F1 (overlap between proposed and gold labels), duplicate precision and recall, needs_human recall (a missed escalation is the costly error), reply-point coverage, $/issue and p95 latency. Coverage uses Day 45's binary judge, one point at a time, reason first. Six gold issues are injection fixtures, so report quality on all 30 and attack success separately.

Single vs Multi-Agent: The Measured Case
R4 is a multi-agent design: a reader, an investigator, a triager and a responder. Day 64 taught you to earn that complexity with numbers. The baseline is one create_agent that sees the raw issue, holds every proposal tool, and passes through the same input guards, output guards, approval and apply path. Offline, GullibleChat stands in for the model. Like Day 69's GullibleModel, it obeys every label order it reads, so its attack numbers show the mechanism, not a real model's behaviour.
# script_id: day_076_capstone_r4_triage/atlas/single_agent
# atlas/single_agent.py: the baseline. ONE create_agent reads the raw issue and holds every proposal
# tool, with the same guards and the same approval + apply path as the pipeline.

Offline, $ is zero and the reply column is the word-overlap proxy, so read only the shape: the gullible single agent copied severity:S1 into a write on all six injection issues, and the pipeline on none.
Write the decision in the README with live numbers. The pipeline's attack success rate (ASR, attack wins ÷ attempts) should be 0% on the six fixtures, because flagged issues get no auto-labels. Enum steering when the flag misses is the residual risk. A live single agent's ASR depends on how well the model resists, with approval as its only backstop, and it may match the pipeline on labels at fewer calls. If repeated live runs show it equal on quality and 0% ASR, say so. Here the security boundary, not quality, justifies four components.
| Decide on... | Pipeline wins when... | Single agent wins when... |
|---|---|---|
| ASR on injection fixtures | stranger text must never shape a write | every write is reviewed anyway and reviewers are careful |
| label quality | facts in the schema are enough | the label depends on details the schema drops |
| $/issue, p95 | the reader and triager are cheap calls | one call with everything is cheaper than four |
Red-Team CI for R4
Day 75's suite judged trajectories. For R4, plain pytest tests judge actions (they need the tracker's write log), and the reviewer is a rubber stamp who approves everything. Whatever survives that worst case is what an attacker can actually do. Fixtures are found by text, never by hard-coded id.

## THREAT_MODEL.md: R4 delta (register from Day 68)
| Id | Status in R4 | Residual risk |
|---|---|---|
| T01 injection via issue | quarantine + no auto-labels on flagged issues | enum steering of kind/component when the flag misses |
| T02 injection via docs/tool output | R3's untrusted wrappers | R3's summary and next_steps are model text that reaches the triager and responder |
| T03 excessive agency | 3 write kinds by type, one write module, approval, scope check | a tired reviewer who approves everything (the rubber-stamp test bounds the damage) |
| T05 off-policy output | output guards + refine cap 2 | regex promise check misses paraphrases |
| T06 unsafe rendering | safe_markdown on every draft | renderer differences on the real tracker |
| T09 PII | redaction before the reader and triager, output guards on every draft, store=False | R3 reads the raw stored issue; names and free-form PII the regexes don't know |
| T11 unbounded consumption | AgentBudget per issue | R3's own cap is token-based, not $-based |
In production
- Serve it behind
/v1/triagewith Day 71's CSP header (Day 77). - Run batches and reports as background jobs (Day 78: Async Jobs & Long-Running Work).
- Use
PostgresSaveronce several workers resume threads (Day 54). - Auto-reject stale approvals with a sweeper, as Day 62 did.
- Watch the approval edit rate weekly: a rising rate is a regression your eval missed.
Go Deeper (optional)
Go deeper: the channels that are still tainted
R4's boundary is strong for the issue body and weaker for everything derived from it. R3 reads the raw issue and writes summary, probable_cause and next_steps as free text. Those strings reach the triager's brief and, through reply_points, the responder. An attacker who can steer R3's wording can steer the reply. Exercise 3 shows the output side holds anyway (safe_markdown drops the link), but the label side relies on the triager's judgement plus code checks.
What experts also do:
- Shrink the free text. Give R3's
Diagnosismore typed fields (a cause enum, doc ids, issue ids) and pass only those to the triager. Each free-text field you remove closes a channel. - Separate the planner from the reader completely. The privileged side builds actions from enums with plain code, which is Day 69's
propose_actions. R4 keeps a model in the triager because labels need judgement; the code checks bound what it can do. - Track provenance. Tag each field with where it came from (issue, doc, model) and refuse to put model-derived text into a write without review. This is the same idea as taint tracking in web frameworks.
- Test the derived channel. Day 75's mutators (base64, HTML comment, role play) applied to R3's output, not only to the issue, find holes the fixture set can't.
Go deeper: approvals that scale
One interrupt per issue with a batch of actions keeps a maintainer's review to one screen per issue. Per-action interrupts are finer but multiply resumes. At ten issues a day the batch shape wins.
Reviewer fatigue is the real T03 risk. The rubber-stamp test bounds the damage when a person approves everything, and two habits keep approvals meaningful. First, sort the queue so needs_human issues and flagged issues come first, with the flag reasons shown. Second, auto-reject anything older than its time to live, as Day 62's sweeper did, because an approval on a week-old proposal approves a stale view of the issue.
Measure the approval edit rate: edits divided by approvals, from audit.jsonl. It's the cheapest live quality signal you have. A rate near zero means either the drafts are good or nobody reads them, so spot-check a sample.
GitHub sandbox mode. To try R4 against a real tracker, create a throwaway repository, copy a few Orbit issues into it, and write a GitHubTracker with the same three methods and the same key argument. Use a fine-grained personal access token limited to that one repository with only issues read and write permission (⚠ verify the exact permission names in GitHub's token settings before you rely on them). Keep idempotency by writing the key into a hidden HTML comment in each comment body and checking for it before posting. Never point it at a repository other people use.
Common Pitfalls
ImportError: cannot import name 'make_chat_model' from 'atlas.llm'after copying files over R3. You replaced R3'sllm.pywith the complete file from this page. On top ofr3-v1.0, append only theR4 additionsblocks toconfig.py,llm.pyandschemas.py.- Approvals "lost" after a restart.
resumeused a differentthread_id, so LangGraph started a fresh run. Thread ids are case files:triage-<issue_id>today and tomorrow. - Duplicate comments after a crash. A write ran inside a node before
interrupt(), so it re-ran on resume. Writes belong inapply, after the pause, keyed by the idempotency key. - Eval numbers too good to be true. The red-team fixtures leaked into quality tuning, or a prompt was tuned on the six injection issues. Report quality on all 30 and ASR separately, and keep new security cases out of
triage_gold. BudgetExceeded: triager: per-issue cap $0.05 reachedin eval runs. R3's web search is on: its cost uses up the cap, and the error names the next agent to reserve budget. KeepATLAS_WEBunset for evals; at $10 per 1,000 calls (as of 2026-11, verify) it dominates the bill.
Key Takeaways
Quick Reference
Capstone R4, on one card
| API / pattern | Use it for | Gotcha |
|---|---|---|
interrupt(payload) + Command(resume=...) | pause for a maintainer, resume later | needs a checkpointer and the same thread_id; the node re-runs from the top |
SqliteSaver(sqlite3.connect(path, check_same_thread=False)) | approvals that survive a restart | separate package langgraph-checkpoint-sqlite |
ProposedAction.make(kind, issue_id, payload) | a write waiting for approval | the key is content-derived: edits keep the original key |
apply_action(..., expected_issue=) | the only write path | denies writes to any other issue |
call_parsed(agent, budget, ...) | every live model call | reserves budget before the call; store=False |
create_agent(model, tools, system_prompt=, middleware=) | the single-agent baseline | always pass a model; ModelCallLimitMiddleware caps calls |
| rubber-stamp reviewer test | bound what an attacker can do | judge actions, not replies |

Warm-up. Add a
needs-infolabel when the quarantined facts have noorbit_version, and a reply point asking for it. Remember the label allowlist.Peek at the approach

Async approvals: run a batch, let the process exit, resume "the next day" from SQLite in fresh processes (twice), and prove from the audit log that every approved action was applied exactly once.
